Basic Level:
- 2 Regular Cleanings** & Exams; $125 each additional Periodontal Maintenance Visit
- Necessary X-Rays
- 1 Fluoride Treatment
- 1 Cosmetic Consultation
- 1 Emergency Care Visit, Exam, & Necessary X-Rays
- No deductibles
- No claim forms
- No one will be denied coverage
Enhanced Level:
- Includes Basic Level Benefits in Addition to the Following:
- 10% Discount for Restorative Treatment
- 25% Discount for Periodontal Treatment
- $500 Off Dental Implant
- $500 Off Orthodontic Treatment or Adult Vivos, $300 Off Child Vivos
- No waiting periods (immediate eligibility)
- No yearly maximum
- No pre-authorization requirements
- No pre-existing condition limitations
Program Exclusions & Limitations
This is a savings plan, not dental insurance. It cannot be combined with any insurance. It is valid only at this dental office. Plan fees are subject to change. For complete details, see Plan Agreement or Plan Terms and Conditions.
*Children 15 or younger **If periodontal infection is present, periodontal therapy will be required before regular periodontal maintenance visits can be provided.Plan Terms and Conditions:
BASIC LEVEL and ENHANCED LEVEL
This is NOT dental insurance. It is a membership-based dental savings plan that is valid only at this dental office. Care from other providers or specialists is not included. This plan is designed to be used by patients who are not using dental insurance, It may not be used with dental insurance. This plan may not be used with other discounts. Enrollment: To enroll, current patients must have an account balance of ZERO. Effective Date: This plan becomes effective on the date when payment is made and remains valid for one full year. It may not be used retroactively for treatment received before the effective date. The member is responsible to use the benefits within their plan year. Any unused benefits will NOT be carried over or refunded. There are no waiting periods. The plan benefits are non-transferable. Refund: The member may receive a full refund within 30 days of enrollment as long as no treatment has been started. Broken Appointments: If appointments are broken without one full business day’s prior notice, a cancellation fee will be charged: $50 for each hour of hygienist time and $150 for each of hour of Dr. Jordan’s time. Adult membership fees are for all patients age 16 years and older. Child membership fees are for patients ages 15 years and younger.
Plan Terms and Conditions:
ENHANCED LEVEL ONLY
Payment for Services: To receive the Enhanced adjustments, payment must be made in full on or before treatment is started. If using “interest-free” 3rd party financing such as Care Credit or Lending Club, the fee adjustment will be 5% instead of 10%, Treatment Fees: Fees quoted on the treatment plan are valid for 90 days. After that time, they may be subject to change. Upgrading to Enhanced Level Plan: To upgrade from the Basic Level to the Enhanced Level, the patient must pay the entire membership fee for the Enhanced Level.
